Incidental diagnosis of a renal angiomyolipoma in an adolescent

Abstract

Introduction: Angiomyolipoma is the most common benign renal tumor. It is predominant in women aged 30 to 40 years and may be asymptomatic or manifest as nonspecific symptoms such as flank pain and hematuria.

Objective: To present the clinical case of an adolescent with an incidental diagnosis of renal angiomyolipoma.

Case presentation: A 17-year-old female adolescent with a personal history of anemia and recurrent urinary tract infections was admitted with a suspected diagnosis of dengue fever due to a fever that had been developing for 4 days. During routine follow-up tests, a complete blood count was performed, which confirmed anemia, and an abdominal and renal ultrasound revealed a tumor image in the upper pole of the left kidney, vascularized, heterogeneous, with a well-defined contour and compressing the adjacent calyces. When a CT scan was performed, the image captured contrast and measured 55x45x46 mm. Therefore, it was decided to consult with urology, who performed a total nephrectomy and a biopsy, which reported renal angiomyolipoma with a spindle cell pattern and infiltrated the capsule.

Conclusions: Renal angiomyolipoma has a low presentation in adolescence, but should be suspected when a renal mass associated with hematuria is detected.
