Application of bioethical principles and quality of doctor-patient communication in decision-making on renal replacement therapy.

Abstract

Introduction: Advanced chronic kidney disease requires complex decisions about renal replacement therapy. The application of bioethical principles and effective communication are fundamental for patient-centered care.

Objective: To evaluate the application of bioethical principles and the quality of doctor-patient communication in decision-making about renal replacement therapy.

Methods: Qualitative field study with a phenomenological approach. Observations of 50 care processes, surveys of 54 patients and 15 professionals were conducted in the Nephrology Service of the Celia Sánchez Manduley University Hospital. Aspects of communication, education, and patient participation were analyzed.

Results: 74% of observations showed a lack of comprehension verification by professionals. 90.74% of patients reported receiving information occasionally or never. However, 79.62% of patients felt they had participated in decision-making about their treatment.

Conclusions: Significant deficiencies in patient communication and education were identified, despite a perception of participation in decision-making. It is necessary to improve the implementation of bioethical principles and the quality of communication in nephrological practice.
